Immunization Schedule In India

According to the Indian Academy of Paediatrics (IAP), it is important to follow the complete immunization schedule to help prevent major illnesses and health risks in your baby.

Here is a list of all the vaccines your baby should be given from birth till age 12.

At birth: BCG, OPV 0, Hep-B 1 – These will be given at the hospital before discharge.

6 weeks:  DTwP 1, IPV 1, Hep-B 2, Hib 1, Rotavirus 1, PCV 1 – This contains the DTaP, Polio and Rotavirus immunization.

Check with your baby’s paediatrician about when to avoid the DTaP vaccine. If your baby has already completed the primary schedule with accellular pertussis (aP) vaccines.

10 weeks: DTwP 2, IPV 2, Hib 2, Rotavirus 2, PCV 2 – If you are giving RV 1, the first dose should be at 10 weeks.

14 weeks: DTwP 3, IPV 3, Hib 3, Rotavirus 3, PCV 3 – If you are giving RV 2, only 2 doses are recommended. In case you give RV 1, the second dose should be at 14 weeks.

6 months: OPV 1, Hep-B 3 – The 3rd and 4th dose of HepB vaccine should not be given before 24 weeks. There should be a gap of at least 16 weeks after administering the first dose.

9 months: OPV 2, MMR-1 – This is the measles, mumps and rubella vaccine. Do not give this before 9 months, and no need to give a separate measles vaccine, as it is already combined in the MMR. The second dose is given at age 2.

9-12 months: Typhoid Conjugate Vaccine – There are 2 types of Typhoid Conjugate vaccine available in the Indian market known as Typbar-TCV® and PedaTyph® and any one can be given. This should be given at a gap of 4 weeks from the MMR.

How A Child’s Ears Get Infected

To understand how your child’s ears can get infected, you should first know more about the air that is present in the middle ear.

The air that is present in the middle part of the air reaches there through tubes known as the Eustachian tubes. These are the tubes that help to connect the middle part of the ear and the back of the throat. Sometimes, the Eustachian tubes also end up transferring things other than air, such as germs.

In many instances, the tubes become a pathway for various germs to move from the throat or the nose and reach the ear area. When these germs reach the ear, they settle in the middle part of the ear which leads to infection and pus. The medical term for this process is otitis media. It refers to the inflammation that takes part in the middle part of the ear and causes the infection.

One of the first signs of otitis media is a simple cold. As your child’s body tries to fight the virus, it leads to swelling in the throat as well as inside the nose. This again leads to stuffiness, which then affects the Eustachian tubes. As a result, the Eustachian tubes are not able to control the germs from reaching to the middle ear and are also not able to throw out any germs that may already be present inside, or might be trying to get in.

Some of the first germs that get inside are mostly the same ones that led to the cold. These ear infections are actually viral infections that are a part of the cold that your child has. Just as your child’s immune system will fight off the cold and treat it in a few days, it will also fight off the ear infection and help it get better on its own soon.

However, in some cases, even when your child’s ear infection gets better, there is a possibility of it happening all over again. The reason this happens is that, once the initial ear infection is better, your child’s immune system is relaxed and the defences are low. This is the time when bacteria that is already present in your child’s body gets active, and reaches inside the middle part of the ear.

When this happens, the ear infection will usually be more serious than the first one. If you see any pus, leakage, swelling or if your child experiences pain, discomfort or hearing problems, visit a doctor immediately.

Helping The Child In Ear Pain

An ear infection can be extremely painful and uncomfortable for your little one. Whatever the cause for the same, it is important that you follow the right care tips at home so that you can help to reduce your child’s discomfort as much as possible.

Here are a few tips you can try to help your child if he or she is suffering from ear pain.

  1. Head up: Try to place your child in such a way that the head is propped up a little, whether while sitting half up or even while lying down. This will help to reduce the amount of pressure that builds up against the eardrum, and can significantly reduce pain.
  2. Hot water therapy: You can use a hot water bottle or even an electric heating pad against your child’s ear, but make sure that you do this only in case of older children. Babies, toddlers and younger kids should not be given the hot water therapy at home, as they are very young and restless, and may end up hurting themselves. If you are using the hot water bottle or pad for your child, make sure you keep touching it regularly to monitor the temperature. Also, do not place it at the same place for long, as it can lead to redness and itching. Do not let your child fall asleep on a heating pad or hot water bottle as it could also lead to accidental burns.
  3. Pain medication: Speak to your child’s doctor to check if it is alright to give some pain medication to offer relief from the pain. If your child is in severe distress, taking a pain medication will help the child to sleep well and recover from some of the discomfort and pain. Make sure you do not get any over the counter drugs but only go by what your child’s doctor has suggested, whether it is an oral medication or an ear drop.

The doctor may prescribe a pain medication to help your child tide through these hours till the medicine starts getting effective.
